This is in addition to the notice issued on July 23, 2014 to general public, holding Card/Wallet, a Prepaid Payment Instrument hereinafter "PPI", having unused balance, be and is hereby given by the issuer of the PPI i.e., M/s Oxigen Services (India) Private Limited (CIN U74999DL2003PTC121961), website: www.myoxigen.com a company incorporated under the Companies Act 1956 having its registered office at 36, 2^nd Floor, Gola Market, Behind Golcha Cinema, Daryaganj, New Delhi - 110002, Delhi, India and its Corporate Office at: 8^th Floor, c/o COOFFIZ, Magnum Towers, Golf Course Extension, Sector 58, Gurugram, 122011, Haryana, India.

The holder of PPI with unused balance issued before 2012 needs to put forward their claim for utilization of the instrument with fresh issuance, to reach us by October 31,2014 at the above said corporate office address of the Company:-

In case of Card: original card having card number in legible form along with any self attested government ID proof of the claimant with mobile number,
In case of Wallet: Wallet ID (mobile number) along with any self attested proof of the claimant that he is the owner of the said mobile number.

In the absence of any claim or incorrect supporting documentation whatsoever within above said stipulated time, the Company will do all the acts as authorized by prescribed regulation including forfeiture of the amount lying unused in the Wallet and card.
